Our program provides healthcare services, transportation, recreational activities, medication management / administration, scheduling and staffing to your clinical appointments - in your residence or ours.
Our program is committed to working with hospital programs, medical laboratories, occupational / physical therapists, social workers, nurses, psychologists, physicians, families and other professionals, as the individual desires or requires.
Our program operates an individualized plan of service that utilizes the person-centered planning and self-determination principles, as it’s core tools coupled with well trained and caring support staff for optimal/beneficial service provisions. |
