| Service Outcome Measures |
| We have developed some great in-house tools that allow our programs to measure and monitor program successes and regression. The measuring tools are based on a combination of The Rensselaerville Institute (TRI) outcome models and Six Sigma methodologies. |
| |
| For example: |
| Service Use Report: |
Answers the question, “How much service has been delivered, and what time and money remain?” Here we report out specific services delivered and the amounts of service that participants have received from each contract or grant. We use this report for billing purposes with funders who pay on a fee for service basis and to identify how much money has been given to a participant (e.g., a utility payment, activity fund or rental assistance). |

| Consumer Demographics Report: |
Answers the question, “Who are we serving?” Here we provide the most common aggregate participant demographic information required by a majority of funding sources. It produces an unduplicated count of participants and families who have received a service in a specified time frame. |
